Eligibility for the COVID-19 vaccine often changes, and is dictated by the government.

In line with government recommendations, proof of eligibility may be required for some patients booking certain vaccines.

If you present to the clinic without proof of eligibility, we may be unable to give the vaccine on the day.

If you are over 60 years old and booking an appointment for the Pfizer vaccine only:

You will need to have your doctor complete this Queensland Health form and bring it with you to your vaccine appointment:

If you are under 40 years old:

You will need to prove your eligibility, and bring to your vaccine appointment either:

  • Proof of occupation
  • Proof of your eligible medical condition*
  • a signed risk vs benefit information sheet (AstraZenca only)**
